We require a proof of a diagnosis or doctors recommendation letter for the use of medicinal marijuana.  Doctors note must contain the doctors office address and name. Sorry we cannot make any exceptions to this. MMAR, MMPR and ACMPR forms are acceptable as well.

2 pieces of  government Photo ID showing that you are of age. We delete all personal ID’s provided after registration is completed. An application/ waiver form signed by you which we will send to you.
